Youth to Adulthood Transitions Program

Program supports youth aging out of care towards independence.

This youth service supports Youth 16-19 in the care of the Ministry of Children and Family Development to transition into independence. The program works with youth to develop a service plan to meet their goals towards independence and connecting to community resources. Program referrals are made by MCFD and VACFSS social workers through the Community Development & Rapid Response Coordinator with the MCFD Youth Connect Team.

Hours

Monday through Friday, 9:00am to 5:30pm. Program hours, however, can be flexible to meet the specific needs of individual participants.

Speak with your social worker.  They will be able to discuss whether the program is a good fit for you.  Your social worker is the only person who can refer you.

Youth Transition Workers will help you to identify your specific goals towards aging out of care and develop a service plan with you.  Workers will then help you to work on your goal areas.  Each service plan is unique to each participant, however some of the general areas we can help you with can include getting necessary ID, employment, life skills, housing search skills, and connection to community resources.

We can work with you until 6 months past your 19th birthday.  Your intake into the program must be completed before you turn 19.
