How To Apply

Referrals are accepted from substance use counsellors (Fraser Health direct service and contracted service providers including Creekside Withdrawal Management) and youth concurrent disorders therapists.

Referrals must be emailed to Shannon Smith, Fraser Health Youth Intake Specialist, at Shannon.smith@fraserhealth.ca

Once completed referrals have been received by email, they will be forwarded to the Fraser Health Screening Committee for review. Screening is typically completed in 24 – 72 hours. Once screened, DEWY staff will contact the youth directly to set up an intake meeting.

Counsellors are available to provide program orientation visits prior to, or during, the referral process.
