The Youth to Adulthood Transition Program provides outreach, counselling, information, programs, and after-hours support to assist youth in developing the life skills needed to successfully live independently.

  • Increased community connectedness

    We also provide outreach in Vancouver to the youth we work with.

WHAT CAN YOU EXPECT?

Youth will work in collaboration with their youth transition worker and social worker in order to achieve the goals they set. They will meet with their youth transition worker on a weekly or bi-weekly basis and will review and update their goals every three months.

Youth transition workers will support youth to gain general life skills on a one-on-one basis. Ultimately, the worker will help participants connect with community resources, and ensure that a young person’s transition into adulthood is successful.

All requests for a youth transition worker are done through the gatekeeper of the program. Please contact Debra MacIver at 604-660-9376 or email her at debra.maciver@gov.bc.ca if you are a social worker and would like to refer your youth to the Youth Transition to Adulthood Program
